Prepare Your Application

In addition to the application form, there are several required supporting documents and a assignment submission that you need to include.

Since you cannot save your form and return, its best to gather what you need in advance.

Application Checklist

  1. Application Form

  2.  Confirmation of graduation (high school transcript, copy of diploma, or other documentation)

  3.  Assignment Submission

  4.  Confirmation of enrollment into your Post-Secondary program (optional) 

  5.  Letter of reference (optional)

Confirmation of Graduation

To confirm your eligibility to enroll in your post-secondary training or program, you are required to submit a confirmation of graduation, or something equivalent such as a copy of your high school transcript, or a GED certificate. In place of a diploma or transcript, a letter from a school official on school letterhead stationery indicating high school graduation and date is acceptable as proof of high school completion.

How to submit: There will be a option to attach this file near the bottom of the form. Confirmation of graduation will be accepted in either a .doc or .pdf format

Confirmation of Enrollment into your Post-Secondary program (optional)

This piece is optional. If you are able to provide proof of enrollment into your post-secondary program or training, it will strengthen your application. However, we understand that not all programs will be able to provide this at the time of application.

How to submit: There will be a option to attach this file near the bottom of the form. Confirmation of enrollment will be accepted in either a .doc or .pdf format.

Letter of Reference (optional)

Another way to strengthen your application is with a letter of reference. This letter could be written by a teacher, employer, supervisor, Elder, Knowledge Keeper – pretty much anyone you could consider a mentor. The purpose of a letter of reference is for someone else to explain why you would be the best candidate for this award.

How to submit: Letters of reference should be no longer than one page. There will be an option to attach this file near the bottom of the form. Letters of reference will be accepted in either a .doc or .pdf format.

Completed Application form with Signature of Consent & Declaration

Once you have determined which materials you would like to include with your application and have prepared your files, you will be ready to fill out the application form.

In order to submit your application form, you will be prompted at the end for a signature of consent and declaration. These signatures are necessary in order to submit the form – if you are under the age of 18, you will need a parent or guardian to sign the form before hitting submit as well.
